Now, facing what the report called Baghdad's "disinterest" in the project, the embassy is gutting what was supposed to be the centerpiece of ongoing U.S. training efforts in Iraq.
According to the report, the embassy plans to turn over the $108 million Baghdad Police College Annex to Iraqis by the end of the year and will stop training at a $98 million site at the U.S. consulate in the southern city of Basra. Additionally, the number of advisers has been cut by nearly 90 percent ? from 350 to 36.
"A major lesson learned from Iraq is that host country buy-in to proposed programs is essential to the long-term success of relief and reconstruction activities. The PDP experience powerfully underscores that point," auditors wrote in a 41-page summary of their inspection. An advance copy was provided to The Associated Press.
Auditors noted that it "has clearly been difficult" for American diplomats to secure a solid commitment from Iraq's government to participate in the training program. Still, the report concluded, "the decision to embark on a major program absent Iraqi buy-in has been costly" and resulted in "a de facto waste."
The findings call into question funding needs at the largest U.S. embassy in the world, as the Obama administration prepares its new spending plan for the 2013 fiscal year that begins Oct 1. While auditors said it's unknown how much the embassy in Baghdad is requesting, additional money for the police program "may not be needed."
Despite years and billions of dollars of training, Iraq's police force remains a vulnerable target for militants. On Sunday, seven police were killed and nine more wounded in bombings and shootings near the former al-Qaida stronghold of Fallujah, about 64 kilometers (40 miles) west of Baghdad. It appeared to be the latest strike by the Sunni insurgency as it seeks to reclaim areas where U.S. troops ousted them.
In a July 26 letter responding to a draft of the report, acting Assistant Secretary of State Carol Z. Perez said the embassy will need an unspecified amount of additional funding this year to continue training Iraqi police into 2013. She disputed the finding that the funds have been wasted, noting Iraqis will continue to use the Baghdad Police College Annex for training.
Moreover, Perez said, the embassy has been assured by Principal Deputy Interior Minister Adnan al-Asadi that Iraq is committed to a streamlined version of the training program. U.S. diplomats will continue working with top security officials, she said, "to ensure that our police assistance efforts meet mutual goals and objectives and to sustain senior-level Iraqi commitment to the program."
The auditors, however, said those assurances fall far short of a written commitment, and quoted al-Asadi as telling U.S. inspectors that the police training program is "useless."
Al-Asadi "also indicated that Iraqi police officers had expressed their opinion that the training received to date was not beneficial," the audit said.
Al-Asadi could not immediately be reached for comment Sunday and his spokesman declined to discuss the report. But a key member of parliament's security oversight committee said that U.S. training programs are no longer needed by Iraqi police.
"The Iraqi federal police went through many training courses, in many fields, and that resulted in having many experts and specialist academies," Shiite lawmaker Hakim al-Zamili said. "At this point, we don't need the American expertise, because of the expertise we have now."
Auditors said the U.S. has spent about $8 billion to train and equip Iraqi police since the 2003 U.S.-led invasion. At that time, there were about 58,000 police in Iraq. The report said that number had grown to 412,000 by 2010. Other estimates put the size of Iraq's federal, local and border police force at 650,000.
The training was led by the American military until last October, just six weeks before U.S. troops left Iraq for good. The embassy took over the program, but with what Monday's report described as "mixed results."
Iraq's self-rule northern Kurdish region has embraced the program and, as a result, half of the remaining 36 U.S. advisers assigned to police training will be based in the Kurdish capital of Irbil, 350 kilometers (215 miles) north of Baghdad.
But restive politics in the central government, whose factions are reluctant to be seen as dependent on American help, have prompted officials to keep the U.S. trainers at arms' length. Some Iraqi officers have been told to skip the police training sessions, the audit said, citing one who blamed "lukewarm relations between the Americans and Iraqis (that) has created some distance between them."
Stephanie Sanok, who was at the U.S. Embassy in Baghdad from 2009 to 2010 and is an expert at the Center for Strategic and International Studies in Washington, called the police training program "doomed from the beginning" because American officials never made sure Iraqis supported it.
"The U.S. government has a tendency to go ahead with programs that it has decreed are in the host country's best interests," Sanok said. "This was such an expensive program, and there was plenty of time to get the Iraqi government to help shape it in such a way that they could eventually take it over. But we never got that buy-in."

WASHINGTON ? An increase in male circumcision in a South African community coincides with lower overall HIV rates among adult men in that population, a new study finds. Meanwhile, a report from Kenya bolsters earlier findings that young men who get circumcised are less than half as likely as their uncircumcised peers to acquire HIV.
The two reports were presented July 24 at the 19th International AIDS Conference.
Researchers in Orange Farm, South Africa, devised a community-wide program in 2008 that provided free circumcision for men, plus condoms and counseling. Within three years, the adult male circumcision rate jumped from 17 percent to 54 percent, said Bertran Auvert, a physician at the University of Versailles in France who works on the project.
Surveys of several thousand men in Orange Farm in 2008 and 2011 found that the prevalence of HIV dropped from 15.4 to 12.3 percent during that time. Auvert calculated that there were 1,040 HIV infections avoided.
The decrease in HIV prevalence, he said, should lead to a decrease in HIV rates for the men?s female partners. ?It?s the beginning of the story, so we?ll need some time,? he said.
An earlier trial in Orange Farm was among the first to show that male circumcision can reduce HIV infection risk. Now, a new analysis of data from a similar trial in Kisumu, Kenya, suggests the protection is long-lasting. Epidemiologist Robert Bailey of the University of Illinois at Chicago reported that 66 months after circumcision, men who underwent the operation were still less than half as likely to become infected with HIV as men who didn?t undergo it.
?After five and half years of follow-up, the fact that these results are consistent over time suggests the protective effect is sustained,? Bailey said.
But the apparent protection provided by circumcision is partial, not total. The foreskin around the penis of an uncircumcised man provides a warm, moist environment that can harbor a virus ? and from which it can invade the body or subsequently be spread to a sex partner. Removing the foreskin by circumcision leaves toughened skin that?s less susceptible to viral infiltration and transmission, Bailey and others have theorized.
Researchers had noticed a link between circumcision and reduced HIV risk when maps of Africa showed higher HIV rates in regions, notably eastern and southern Africa, where male circumcision wasn?t performed as a traditional practice (SN: 1/3/09, p. 14). Before the first circumcision trials in Africa were started about a decade ago and results from those tests were made public, many people in Africa ?thought that circumcision equated to becoming a Muslim,? said Samson Kironde, a physician in Jinja, Uganda, at JSI Research & Training Institute and USAID. The community-wide effect seen at Orange Farm ?is significant,? he says, ?and that?s the aim.?
Kironde says that by some estimates, circumcising 80 percent of men in a community ?would allow you to reach a critical mass of people? that would have a potent community-wide effect.
Now there are hints that male circumcision is gaining steam in these regions as a lifestyle choice.
A survey of 101 Kenyan women whose male partners had been circumcised found that all the women were satisfied with the choice and that 91 percent reported enjoying sex more since the operation. Timothy Adipo of Nyanza Reproductive Health Society in Kisumu, however, reported that one-third of the women also said condom use was less necessary since circumcision. ?They perceived themselves at lower risk,? Adipo said. ?Our educational message needs prioritizing now.?

MADRID (AP) ? France and Spain called Wednesday for swift implementation of the rescue plan for Spanish banks to ease concern that the country may need a full-blown sovereign bailout.
In June, the 17 countries that use the euro agreed to lend Spain up to ?100 billion ($121 billion) to save those of its banks that are laden with soured investments following a property sector collapse in 2008.
But the deal only intensified concerns about Spain's debt levels, since the government initially will have to repay those loans if the banks cannot. In response, investors have since been demanding exorbitantly high interest rates to lend to the country.
To solve that problem, eurozone ministers agreed to give the loans directly to the Spanish banks ? but only after a Europe-wide banking regulator is set up. That could take months, if not years. France and Spain said this step should be sped up and completed by the end of the year.
"The swift implementation of the financial assistance programme is essential to restore confidence and recreate conditions for growth," French Finance Minister Pierre Moscovici and Spanish Economy Minister Luis de Guindos said in a joint statement after talks in Paris.
The ministers said they did not think Spain's borrowing costs, which this week touched a euro-era record, reflect the country's financial position.
"We believe that the current level of interest rates prevailing in the sovereign debt markets does not reflect the fundamentals of the Spanish economy, its growth potential and the sustainability of its public debt," the statement said.
Both men said they were confident the rates would fall back once investors were confident that the country's banks were getting the support they need.
In the markets, however, Spain remained under intense financial pressure.
The yield, or interest rate, on Spain's benchmark ten-year bond spiked 0.11 percentage point to 7.65 percent in the first hour of trading, before easing back to 7.38 percent after the statement's release mid-afternoon.
A rate above 7 percent is deemed untenable over the long term ? Spain has been suffering it for several weeks. Should Madrid find it too expensive to raise money from bond markets at such rates, it would have to ask for an international bailout like those sought by Greece, Ireland or Portugal.
Spain denies it will need financial rescue for its public finances, but many investors now think it's only a matter of time.
The country's benchmark Ibex 35 stock index was up by a half percent, recovering only a fraction of the previous three days' heavy losses.
De Guindos' visit to Paris is part of the country's desperate battle to seek support and stave off a bailout. He met with Germany's Wolfgang Schaeuble in Berlin on Tuesday. Spain has also been pleading for the European Central Bank to intervene and buy its debt to ease get its bond yields down. But so far its demands have fallen on deaf ears.
Spain's economy is in its second recession in three years, chiefly because of a collapse of the property bubble that had fueled the economy for more than a decade. It is not expected to grow again before 2014 and unemployment is around 25 percent.
Meanwhile, two regions ? Valencia and Murcia ? have since Friday said they will tap an ?18 billion emergency credit line the central government recently set up for cash-strapped regions. More are expected to follow.
The powerful northeastern region of Catalonia said Tuesday it was also considering requesting help from Madrid, but stressed that the credit line did not constitute a rescue or bailout for the region.
Catalonia, with a debt of ?42 billion, is Spain's most indebted region but also one of the top two contributors to the nation's GDP.
Many Spanish regions are so heavily in debt due to the recession and the real estate crisis that they cannot raise money on their own on financial markets.
Investor concern about regional debt grew sharply when the central government was forced to revise Spain's 2011 budget deficit upward for a second time to 8.9 percent in May ? an embarrassing adjustment that had to be made after four of the regions confirmed they had spent more than previously forecast.

Practice Administrative Manager Title: Practice Administrative Manager Location: Lenox Hill Hospital Other Locations: The Practice Administrative Manager supports and is responsible for the daily administrative operations of a multi-physician, single and multiple practices for a division or program. In this role, you must be able to coordinate administrative functions, plan and support development of the site and joint division programs and all related areas.You will manage all non-clinical personnel, maintain accurate and current files on all department budgets, capital equipment requests and programs related to operational issues. It is important that you maintain and encourage efficient and positive communication with team members in order to foster a pleasant environment. You will also support revenue cycle functions, Quality and Service Excellence initiatives. Responsibilities Include: 1. Coordinates administrative functions, plans and supports program development of the Department/Division. * Coordinates activities including growth objectives, space allocation, procurement of technological equipment, data planning, developing goals and objectives, capital requirements and desired outcomes of programs. * Serves as a liaison with Administration in planning, developing, implementing and evaluating operations. * Reviews comprehensiveness of programs on an ongoing basis and develops strategies for program adaptation to market changes. * Prepares reports and documents for Certificate Of Need (CON) applications. Plans and coordinates construction projects. * Monitors program components relative to criteria by regulatory agencies. * Collaborates with the Research Institute to oversee Research operations. 2. Manages department/division in a fiscally responsible manner. * Assists in the preparation and monitoring of annual budgets - operational, personnel and capital. * Supports or may assist with key revenue cycle functions including, but not limited to, scheduling, insurance verification, co-pay collection, charge capture and referral management. * Understands the department/division's fiscal responsibility in supporting research initiatives. 3. Manages daily operations and all non-clinical personnel in the division and all related areas. * Hires, trains, disciplines and supervises staff. Evaluates performance and ensures efficiency in their roles. Completes appraisals on a timely basis. * Oversees scheduling (vacation, holidays) and ensures adequate coverage at all times. Performs related personnel functions. * Ensures staff members understand their roles and responsibilities. * Supports continued growth of staff and offers opportunities for advancement. * Assists in developing and maintaining objectives, policies and procedures to ensure the efficient operation of the department/division. * Strives to streamline operations and effectively apply new concepts and techniques for positive outcomes. * Identifies and implements technology to improve operations. * Assigns work and sees that it is carried out correctly and efficiently. Coordinates and oversees the resources of multiple administrative disciplines/functions. * Coordinates efforts to prioritize needs of programs across departmental lines. * Collaborates with administrative leadership, chairs, physicians and other clinical personnel to develop, implement and oversee clinical programs. * Participates in monitoring and evaluating a program's inception and performance and routinely monitors performance per volumes, referrals and trends. * Optimize billing practices and collection of Part B revenues. * Coordinates physician schedules pertaining to division services. * Keeps abreast of all billing, coding and reimbursement related issues as they apply to the division and facilitates staff education in regard to these changes. * Structures the appropriate flow of data to insure timely and appropriate management of clinical and financial issues, which may comprise patient care in the department. * Establishes and maintains a working relationship with the physician staff with respect to their compliance and performance related to billing, coding and regulatory agency standards. 5. Ensures the continual efficient and compliant operational performance of the department/division. * Compliant with all JCAHO, NYS DOH, Public Health Law, Institutional and Departmental rules and regulations and makes certain these are properly and completely addressed in Policy and Procedure Manual. * Reviews all changes in various regulatory agency standards and/or public health law and ensure that these are represented in the Policy and Procedure Manuals (after appropriate review with clinical and/or administrative personnel). * Ensures department/division staff adheres to system Corporate Compliance Program, HIPAA regulations and all other regulatory standards. * Identifies quality benchmarks utilizing regional and national data to monitor programmatic quality and identify opportunities for improvement. * Bachelor's Degree in Business Administration, Health Care Administration or related field, required. * Master's Degree, preferred. * Minimum of three (3) years relevant managerial experience in a hospital setting required; experience in off-site practice management preferred. * Extensive knowledge of the health care marketplace, financial analysis, current reimbursement issues, budgets and operational analysis, required.
